The undersigned applicant's parent/guardian give permission for representatives of the sending school to release the applicant's records including grades, attendance, and conduct/discipline records with this application. It is understood that upon acceptance, official transcripts, current IEP's, 504 Plans, and results of standardized testing completed over the last three (3) years will be required.
