Official Transcript Request

CURRENT STUDENTS: Fill out the Transcript Release Form located in the Guidance Office.

FORMER STUDENTS: A request for a transcript must be in writing, either by mail or by fax, and must include the following:

Name
Date of Birth
Dates of Attendance and/or Graduation Date
Full address where the transcripts are to be sent. If being picked up, please specify WILL PICK UP.
A telephone number you can be reached at.
Your signature
